If you are found to be eligible for TASC case management services, there is also a $100 case management fee. Payment arrangements can be made for the case management fee.
If you can’t afford the $80 assessment fee and believe that your household income is below 100% of federal poverty guidelines, you must bring proof of household income to be considered for a fee waiver. You must bring documentation of the entire household income with you to prove that your household income is below 100% of federal poverty guidelines; otherwise, you need to have $80 to be seen. Acceptable documentation is the last two pay stubs for all wage earners in the household, proof of social security benefits for all members of the household that receive this benefit, proof of an open Dept. Of Jobs and Family Service case, an active Medicaid card, and/or proof of alimony or child support.
